Poor health, disability and loss of independence are not the inevitable consequences of aging and chronic conditions.  However, continuing to live healthy as you age and develop chronic conditions requires a plan.  You can manage symptoms, medicines, medical expenses and difficult emotions if you have support with occassional problems and questions.

CPC clinicians, counselors and technicians team with patients and their primary care practitioners to develop chronic care plans, and support them with monthly visits, education, and other tools.  Some key measures of such a plan are:

  • Practicing a healthy lifestyle.
  • Managing symptoms and difficult emotions.
  • Detecting chronic conditions before they limit funtion.
  • Preventing medication-related problems.

When patients and practitioners need solutions or support outside our scope of practice, we help locate other resources in their communities.
